OFFICIAL RULES

Florida Vision Lasik is sponsoring a Free Lasik Procedure giveaway promotion Promotion:

The promotion begins on February 14, 2009 and continues through May 31st, 2009 (“Promotion Period”).

Prize: A Bi-Lateral LASIK procedures with an approximate value of $6,000 (the “Prize”) will be awarded to one Winner during the Promotion Period. The odds of winning depend on the number of eligible entries received.

Sponsor reserves the right, in its sole discretion, to substitute a comparable service in the event the Prize is unavailable.

Rules and Additional Information

1. 1. Promotion is limited to one entry per person
2. 2. To qualify, participants must complete a complimentary consultation to determine their candidacy for LASIK.
3. 3. All participants who have a vision correction procedure performed by Florida Vision Lasik on or before 5/31/2009, will be entered into a random drawing to receive a full refund of their LASIK procedure fee. Winner will be notified within 15 days following the end of the Promotion.
4. 4. Odds of winning depend on the number of eligible entries received.
5. 5. Contestants must be 18 years or older to enter and win.
6. 6. No substitutions or case redemption of prizes will be allowed.
7. 7. Winners will be selected in a random drawing from all entries received for the contest. Winners will be notified by e-mail and telephone.
8. 8. By entering this giveaway, each entrant agrees to be bound by these official rules and to comply with all federal, state and local laws and regulations.

Entries will be void if incomplete, forged or otherwise not in compliance with these official rules. The providers for this giveaway make no warranties, and hereby disclaim any and all warranties, express or implied, concerning any prize furnished or made available in connection with this giveaway.

9. 9. By entering this giveaway, entrants release and hold Florida Vision Lasik, it’s owners and employees, harmless from and against any and all losses, damages, rights, claims and actions of any kind arising in whole or in part, directly or indirectly, from the giveaway or participation in any contest-related activity.
10. 10. Employees of Sponsor, its affiliates, divisions, subsidiaries,advertising/promotion agencies, or any third part associated with the development or administration of this Promotion, and members of such employee’s household and/or immediate family (i.e., parents, children, siblings, spouse and persons residing in the same household as such immediate family members) are not eligible for this promotion. Florida Vision Lasik, its affiliates and their respective employees, shareholders, directors, and agents are also ineligible participants. Winning a Prize is contingent upon fulfilling all requirements set forth herein. Prize Winners will be required to sign and return an Affidavit of Eligibility and Liability/Publicity Release (including his/her Social Security Number for tax purposes.) Failure to sign and return these and other such requested documents by the return date specified in the Prize notification letter will result in forfeiture of the Prize. Winners receiving Prizes with Approximate Retail Value of $600 or more will also be required to complete an IRS Form W-9, and will be issued an IRS Form 1099 in the amount of the Approximate Retail Value of the Prize.
